Myself and a group of 250 menopause providers responded to the Lancet medical journal reporting that we are “overmedicalizing” menopause. These remarks were infuriating and damaging to women, clinicians’ researchers in our field. I agree completely menopause (similar to pregnancy) is not a disease. Both reproductive states have symptoms and signs that have ripple effects in every body system that affect chronic disease, wellbeing and longevity.

What We Know
Following the Women’s Health Initiative Study in 2002 linking ( hormone therapy) HT to breast cancer and heart disease, most women discontinued HT and 2 decades of women and health care providers did not recommend HT. Currently less than 5% of menopausal women are prescribed and taking HT.
What’s New
A study published in the respected Journal of the Menopause society, conducted a retrospective (look back analysis of Medicare records) for 10 million women over age 65 who were prescribed HT compared with those who were not over a 14 year follow up (2007-2020) . All routes of administration from oral to transdermal were included. Overall, compared to never use or discontinuation of HT after 65 yr. the use of estrogen alone (in women with hysterectomy) was associated with significant reduction in death (19%) , breast cancer (16%), lung cancer (13%) , colorectal cancer (12%), congestive heart failure (5%), atrial fibrillation (4%), heart attack (11%) and dementia (2%).
For women using combined estrogen and progestin, the risk of breast cancer increased by 10%-19%, but such risk can be mitigated using low dose of transdermal or vaginal E+ progestin. E+P reduced risk of endometrial cancer (45%) ovarian cancer (21%), ischemic heart disease (5%), CHF (5%), and venous thromboembolism (5%), CHF (4%).
What Does This Mean for Me?
This study re-emphasizes the importance of individualizing therapy for each women including women who are further from the menopause transition. Most insurance companies and misinformed clinicians quote the antiquated rhetoric of using HT for 5 years or which clearly for most women including those over age 65 years does not support the evidence for cardiovascular disease and cancer prevention, not to mention bone health. ( I couldn’t help myself!)
